  • Properly filled up Housing Loan Application
  • Insurance form
  • Photocopy of Marriage Contract (if married)
  • Government Issued Identification Card

  • Certificate of Employment with Compensation
  • Latest Crew Contract (for seamen)
  • Latest six (6) months Proof of Monthly Remittances or payslips
  • Special Power of Attorney authenticated by the Philippine Consulate
  • Proof of Billings
  • Bank Statements

SELF EMPLOYED:

  • Business Background / Company Profile (if applicable)
  • Certificate of Business Registration with DTI
  • Copy of Lease Contract and title for income from rented / leased properties
  • Copy of SEC Registration, Articles of partnership
  • Copy of SEC Registration, Articles of Incorporation / By-Laws
  • Audited Financial Statements for the past two (2) years.
  • Income tax Return for the last two (2) years
  • Latest six (6) months Statement of Bank Account / Photocopy of Passbook
  • At least three (3) major Suppliers / Customers with contact information
  • Proof of Billings
